💰 Expense / Reimbursement Request

Use this form when:

• You bought something for the PTA and need to be reimbursed
• A check or payment needs to go to a vendor
• The PTA is making a donation to the school (attach the donation receipt form)

🏦 PTA Deposit Form

Use this form any time PTA money is deposited:

• Cash and checks from events or fundraisers
• Online or electronic payments (Venmo, Square, PayPal)
• Two people must count all cash together before it is deposited
